Everywhere.care is required by law to maintain the privacy of your PHI and is hereby providing you with this Notice of Privacy Practices with respect to PHI and reserves the right to change the terms of this Notice of Privacy Practices at any time as long as the changes are in compliance with applicable law. Any new Notice of Privacy Practices will be effective for all PHI that we maintain at that time. Uses and Disclosures of PHI
The ways Everywhere.care may use and disclose PHI about you:
- Treatment, Payment and Healthcare Operations Everywhere.care is permitted to use and disclose your PHI for purposes of (a) treatment, (b) payment and (c) healthcare operations. For example:
- Treatment Your PHI may be used and disclosed without authorization by those who are involved in your care for the purpose of providing, coordinating, or managing your health care treatment and related services. This includes consultation with other treatment team members or in connection with the provision of follow-up treatment.
- Payment Everywhere.care may use and disclose PHI without authorization so that we can receive payment for the treatment services provided to you. Payment related activities include the determination of service membership eligibility, processing claims, reviewing services provided to you to determine medical necessity, or in utilization review activities.
- Healthcare Operations Everywhere.care may use and disclose your PHI in connection with its healthcare operations, such as providing customer service and conducting quality review assessments. Everywhere.care may engage third parties to provide various services. If any such third party must have access to your PHI in order to perform its services, Everywhere.care will require that third party to enter an agreement that binds the third party to the use and disclosure restrictions outlined in this Notice of Privacy Practices.
- Authorization Everywhere.care is permitted to use and disclose your PHI upon your written authorization, to the extent such use or disclosure is consistent with your authorization. You may revoke any such authorization at any time.
- As Required by Law Everywhere.care may use and disclose your PHI to the extent required by law.

Disclosing PHI for Unique Circumstances
The following categories describe various unique circumstances in which Everywhere.care may use or
disclose your PHI:- Public Health and Threat to Health or Safety If required, Everywhere.care may use or disclose your PHI for mandatory public health activities to a public health authority authorized by law to collect or receive such information for the purpose of preventing or controlling disease, injury, or disability, or if directed by a public health authority, to a government agency that is collaborating with that public health authority. Everywhere.care may disclose your PHI if necessary to prevent or lessen serious and imminent health to the health or safety of a person or the public. If information is disclosed to prevent or lessen a serious threat it will be disclosed to a person(s) reasonably able to prevent or lessen the threat, including the target of the threat.
- Specialized Government Functions Everywhere.care, may in certain situations, use and disclose PHI of persons who are, or were, in the Armed Forces for purposes such as ensuring proper execution of a military mission or determining entitlement to benefits. - Law Enforcement Everywhere.care may, under certain circumstances, disclose your PHI to a law enforcement official, such as for purposes of identifying or locating a suspect, fugitive, material witness or missing person.
- Worker’s Compensation Everywhere.care may disclose your PHI as authorized by, and to the extent necessary to comply with, workers’ compensation programs and other similar programs relating to work-related illnesses or injuries.
- Decedents Everywhere.care may, under certain circumstances, disclose PHI to coroners, medical examiners, and funeral directors for purposes such as identification, determining the cause of death and fulfilling duties relating to decedents.
- Organ Procurement Everywhere.care may, under certain circumstances, use or disclose PHI for the purposes of organ donation and transplantation.
- Research Everywhere.care may, under certain circumstances, use or disclose PHI that is necessary for research purposes.
- Medical Emergencies Everywhere.care may, under certain circumstances, use or disclose your PHI in a medical emergency situation to medical personnel in order to prevent serious harm.

YOUR PHI AND YOUR RIGHTS
You have the following rights regarding the PHI maintained by Everywhere.care: To exercise any of these rights, please submit your request in writing to the Everywhere.care Privacy Officer at 190 West Park Dr., Suite 430, Brentwood, TN 37027.
- Confidential Communication You have the right to request that Everywhere.care communicate with you about health matters in a certain way or at a certain location. We will accommodate reasonable requests.Everywhere.care may require information regarding how payment will be handled or specification of an alternative address or other methods of contact as a condition for accommodating your request.
- Restrictions You have the right to request restrictions on certain uses and disclosures of PHI for treatment, payment or healthcare operations. You also have the right to request that Everywhere.care restricts its disclosures of PHI to only certain individuals involved in your care or the payment of your care. You must submit your request in writing to Everywhere.care. Everywhere.care is not required to comply with your request. However, if Everywhere.care agrees to comply with your request, it will be bound by such agreement, except when otherwise required by law or in the event of an emergency.
- Inspection and Copies You have the right to inspect and copy your PHI. You must submit your request in writing to Everywhere.care. Everywhere.care may impose a fee for the costs of copying, mailing, labor, and supplies associated with your request. In certain limited circumstances, Everywhere.care may deny your request to inspect and/or copy your PHI. If that occurs, we will inform you of the reason for the denial, and you may request a review of the denial.
- Amendment You have a right to request that Everywhere.care amend your PHI if you believe it is incorrect or incomplete, and you may request an amendment for as long as the information is maintained by Everywhere.care. You must submit your request in writing to Everywhere.care and provide a reason to support the requested amendment. Everywhere.care may, under certain circumstances, deny your request by sending you a written notice of denial. If Everywhere.care denies your request, you will be permitted to submit a statement of disagreement for inclusion in your records.
- Accounting of Disclosures You have the right to request an accounting of certain of the disclosures that Everywhere.care makes of your PHI. Everywhere.care may charge you a reasonable fee if you request more than one accounting in any 12-month period.
- F. Breach Notification If there is a breach of unsecured PHI concerning you, we may be required to notify you of this breach, including what happened and what you can do to protect yourself.
- Paper Copy You have the right to obtain a paper copy of this Notice from Everywhere.care at any time upon request. To obtain a paper copy of this notice, please contact Everywhere.care at 1-844-267-8779.
- Complaint You may complain to Everywhere.care and to the Secretary of the Department of Health and Human Services if you believe that your privacy rights have been violated.
